SASI-S Surgery

What is SASI-S surgery?

Single Anastomosis Stomach-Ileal Bypass (or SASI-S Bypass) is a combination of a sleeve gastrectomy and an intestinal bypass. A sleeve gastrectomy is performed, followed by joining the bottom part of the stomach to the distal part of the small intestine. The length of the bypass is determined by the required amount of weight loss.

This results in half of all food consumed bypassing the first part of the small intestine, reducing the absorption of fats and sugars. Slow digestion stimulates the release of intestinal hormones, signalling satiation.

Advantages

  • Greater weight loss when compared to a standard sleeve gastrectomy, or a traditional gastric bypass
  • Less risk of nutritional deficiencies
  • Good quality eating
  • Continuity of gastrointestinal tract is maintained
  • 70% to 90% diabetes control
  • Access to bile duct remains, future gallstones can be easily removed
  • Less risk of an internal hernia
  • Reversible

Disadvantages

  • Increased risk of bile reflux into the stomach
  • Weight loss may be less than SADI-S or One Anastomosis Gastric Bypass
  • Risk of ulcer at stomach and small intestine join
  • Relatively new operation with limited long term outcomes
